It&#8217;s still not clear if this strain of the flu will &#8216;<a href=\"https:\/\/www.nature.com\/nature\/journal\/v439\/n7073\/full\/439124a.html\">evolve the ability to spread between people<\/a>,&#8217; but if this happens, we&#8217;re in for some serious problems:<\/p>\n<p><a href=\"https:\/\/www.who.int\/csr\/disease\/influenza\/pandemic10things\/en\/index.html\"><\/p>\n<blockquote><p>Supplies of vaccines and antiviral drugs \u2013 the two most important medical interventions for reducing illness and deaths during a pandemic \u2013 will be inadequate in all countries at the start of a pandemic and for many months thereafter &#8230; <span class=\"caps\">WHO<\/span> has used a relatively conservative estimate \u2013 from 2 million to 7.4 million deaths \u2013 because it provides a useful and plausible planning target. This estimate is based on the comparatively mild 1957 pandemic. Estimates based on a more virulent virus, closer to the one seen in 1918, have been made and are much higher.<\/p><\/blockquote>\n<p><\/a><\/p>\n<p>The current syntheses of oseltamivir (a.k.a. <a href=\"https:\/\/www.fda.gov\/cder\/drug\/infopage\/tamiflu\/default.htm\">Tamiflu<\/a>) fall &#8216;<a href=\"https:\/\/pubs.acs.org\/cgi-bin\/asap.cgi\/jacsat\/asap\/html\/ja0616433.html?emFrom=emLogin\">short of the ideal for several reasons: (1) the starting point in the synthesis is either (-)-shikimic or (-)-quinic acid, which are complex relatively expensive and of limited availability, and (2) two steps involve potentially hazardous and explosive azide-containing intermediates<\/a>.&#8217;<\/p>\n<p>But two new syntheses of oseltamivir\/Tamiflu were reported in <a href=\"https:\/\/pubs.acs.org\/journals\/jacsat\/index.html\"><em><span class=\"caps\">JACS<\/span><\/em><\/a> earlier this week: one is by <a href=\"https:\/\/pubs.acs.org\/cgi-bin\/abstract.cgi\/jacsat\/asap\/abs\/ja0616433.html\">Yeung <em>et al.<\/em><\/a> and the other is by <a href=\"https:\/\/pubs.acs.org\/cgi-bin\/abstract.cgi\/jacsat\/asap\/abs\/ja061696k.html\">Fukuta <em>et al.<\/em><\/a><\/p>\n<p>Although the synthesis from Corey&#8217;s group is much shorter, both syntheses avoid using either (-)-shikimic or (-)-quinic acid as starting materials: Corey&#8217;s synthesis starts with 1,3-butadiene and trifluoroethyl acrylate and Shibasaki&#8217;s synthesis starts with the asymmetric ring-opening reaction of a <em>meso<\/em>-aziridine with TMSN3. If these syntheses could be scaled up and optimized further, it might be possible to reduce the cost of oseltamivir\/Tamiflu and pharmaceutical companies (or paranoid graduate students) could make plenty of the compound, which could be <a href=\"https:\/\/www.bt.cdc.gov\/stockpile\/\">stockpiled<\/a> and used if a human pandemic occurs.<\/p>\n<p>The only catch: <a href=\"https:\/\/content.nejm.org\/cgi\/content\/abstract\/353\/25\/2667\">oseltamivir-resistant virus has already been reported in Vietnam<\/a>&#8230;<\/p>\n<p>Joshua<\/p>\n<p>Joshua Finkelstein (Associate Editor, <a href=\"https:\/\/www.nature.com\/nature\/index.html\"><em>Nature<\/em><\/a>)<\/p>\n","protected":false},"excerpt":{"rendered":"<p>With H5N1 avian influenza gradually making its way across the world, a lot of people are preparing for the worst.
